Would it be easier for you if we used your conversion rather than the one I did?
I used git cvsimport -a -kk -u -A with this cvs-authors list:

wichert=Wichert Akkerman <wichert at deephackmode.org>
pavel=Pavel Machek <pavel at ucw.cz>
bacchus=Ralf Baechle <ralf at linux-mips.org>
hughesj=John Hughes <john at Calva.COM>
kratochvil=Jan Kratochvil <jan.kratochvil at redhat.com>
nate=Nate Sammons <nate at users.sourceforge.net>
ldv=Dmitry V. Levin <ldv at altlinux.org>
wakkerma=Wichert Akkerman <wichert at deephackmode.org>
ludvigm=Michal Ludvig <mludvig at suse.cz>
vda_linux=Denys Vlasenko <dvlasenk at redhat.com>
roland=Roland McGrath <roland at redhat.com>
drepper=Ulrich Drepper <drepper at redhat.com>
